Report a suspected side effect

Know what to do if an Ayurveda medicine, herb, cosmetic or wellness product may have caused harm or has a quality problem.

Get urgent help first

For difficulty breathing, collapse, severe swelling, heavy bleeding, poisoning, a severe allergic reaction or immediate danger, contact local emergency services. Do not wait to complete a report.

What to record

A useful report connects the person, product, timing and outcome. Do not delay medical care while collecting details.

  • Product name, manufacturer, batch or lot number and a label photo
  • Ingredients, dose, route, start date and when the problem began
  • Other medicines, herbs, supplements and relevant health conditions
  • Symptoms, treatment received, outcome and healthcare contact details
  • Where and when the product was purchased

Where to report

Report to the treating clinician and the seller or manufacturer. The appropriate regulator depends on the product category and your country. Ayurveda.Fit will only accept reports after a secure case-management and clinical-review service is operational.
